    a. Should be hand-signed and dated within the last 3 months

    b. LOA should be in POLISH

    c. The local address on the LOA Should match the area 
       code of the porting numbers.

Copy of Bill

Hard copies of LOA in Polish, hand signed by the customer,
have to be sent to Zooms’ vendor.

NOTE: Customers are required to port in number blocks and any numbers associated with the Main Billing Number.
