Letter of Authorization (LOA)

     a. Should be hand-signed and dated within the last 3 months
    
     b. Account number and stamp in the LOA (if the end-user 
        is a company)

Copy of Bill -  dated within the last 3 months

Additional Requirement: 

     a. For Business: Copy of Authorized representative's photo ID

     b. For Residential: Copy of end-user photo ID

     c. It is mandatory to provide the ID number or passport
        number of the end-user as well as a copy 
        of the document.
