Letter of Authorization (LOA)

     a. LOA Should be hand-signed and dated within the 
        last 3 months. For business end-user, a company 
        stamp is required.

     b. Single numbers are to be presented on separate 
        LOA to DDi blocks. They cannot be combined onto 
        the same LOA. (If you have a broken range)

     c. The local address on the LOA Should match the area
        code of the porting numbers.

Copy of Bill dated within the last 3 months

Additional documents

     a. For Business end-user

          1. Proof of Registration (Handelsregisterauszug 
             or Bescheinigung der Gewerbeanzeige)

          2. Company stamp on the LOA is mandatory

          3. Copy of ID (National ID card or Passport)

          4. Proof of address

NOTE: If your numbers are currently provided by a reseller, please ask your reseller to let you know the losing carrier name in Germany as this is the supplier name that will need to be noted on your

Updated Proof of Business (Any of the following)

• Handelsregisterauszug (Commercial Register)
• Bescheinigung der Gewerbeanzeige (Business Registration)
• Trade license (Gewerbeanmeldung)
• Labor leasing agreement (Arbeitnehmerüberlassung)
• For an NGO (Non-business entity), a Tax Declaration will suffice
